There are two versions of the COYBL registration form:
The Microsoft Word version allows you to fill out
the form on your computer.

HOW TO USE THE NEW MICROSOFT WORD REGISTRATION FORM
The new registration form consists of form fields in which
you can type information. Click the gray boxes to start
typing in each field.
Save the document onto your computer at any time. Print
the completed form and bring it with you to registration.
If you are not able to view the Word version,
download and print the PDF version.
